You and Your Loved One
Your team starts with you and your loved one. You take the lead with day-to-day care, and we’ll be there to guide and support you.

Primary Care Provider
You may choose your loved one's primary care provider to work alongside the hospice care team.

Hospice Physician
A hospice physician coordinates care and works closely with the hospice team to manage your loved one’s symptoms. They may also visit, based upon need.

Social Worker
Social workers provide emotional support and counseling and can connect you with community resources. They can also help you complete an advance directive, such as a living will.

Nurse Case Manager
Your nurse case manager coordinates care and monitors the management of your loved one's pain and symptoms. They’ll answer questions and coach you on caregiving strategies.

Certified Nursing Assistant (CNA)
CNAs are trained caregivers who provide personal care like bathing and shaving.

Chaplain
Our chaplains offer non-denominational spiritual support. They can also help with funeral planning, if requested.

Trained Volunteers
Caring volunteers provide respite care and various other services, including being a companion to your loved one and helping with household chores.

Bereavement Counselor
Loved ones may experience grief before, and after a loss. Licensed bereavement counselors provide individual and group grief support. Our counselors will keep in touch for up to 13 months after a loved one's death.

Levels of Hospice
We understand the different levels of hospice care needed for each person and their family members during this time. In many cases, hospice care is available earlier than you think. Don’t hesitate to call us if you feel you or your loved one would benefit from hospice care.

You may want to manage much of your loved one's day-to-day care without help. But you don't have to do it all alone. Our hospice care team supports you with guidance and advice. We’ll visit when you need us to help you meet your loved one’s needs.
- Respite Care

No matter how much you love a person, caregiving can be overwhelming. A short break can make a big difference. Let us give you short-term help by admitting your loved one to a contracted facility where they'll be cared for while you rest for a maximum of five days. Our hospice team will also continue to visit your loved one during this time.
- Crisis Care

At times, your loved one’s symptoms may become severe, but you don't need to feel helpless or alone. Designed for short periods, crisis care assists with managing uncontrolled symptoms. Whether remaining at home or moving temporarily to an inpatient unit, your loved one will receive close observation. This level of care is evaluated frequently to determine necessity.

At AdventHealth Hospice Care, we provide inpatient care to patients needing advanced pain control or acute symptom management. Our goal is to stabilize your loved one’s comfort level so that they can transition back to their current residence.
